Obama was the leader of the party. If he said "this is my policy" that is what would have reached the floor. 
Yah, no. That's not actually how legislation (the sausage) is actually made. Pelosi knew exactly how many votes she needed and what she could get away with.

She also knew the bill would not become law without Obama’s signature

By the time it hit the floor, republicans had had a long time to paint it as "death panels" and all sorts of nonsense. The people who voted against it were the super right wing democrats who voted against because their right wing constituents believed the lies about it. If he had picked an actual progressive system that didn't have so many confusing, ridiculous features, it would have been much easier to implement.
