- Some people requesting sex transformation are borderline or overt psychotics, and it may be hazardous to change their bodies to a radical degree.
- Some are quite depressed. Are they depressed soley because they cannot stand living the wrong sex-role or for other reasons?
- Some are primarily homosexual or fetishistic transvestites. For them, the genitals are essential to maintain this level of functioning and any surgical alteration would affect their pleasure.
- Some are homosexual and wish the operation to facilitate prostitution.
Implicit in these points is the unresolved question of the degree to which a change in bodily configuration resolves internalized psychological problems. More specifically, if a change occurs in personality functioning, is it on a conscious, preconscious, or unconscious level? To appraise this requires clinical expertise of the highest order and a good deal of time. The complexity and spectrum of human motives belies any simple answer that could be given on a few brief contacts. The predominance of males seeking this change - four or five males to one female - raises developmental questions."
